General Education Waiver/Substitution Form is available in the Registrar's Office, first floor of the DeWitt Center or on-line. You should complete a form any time you need to request a substitution or waiver of a General Education requirement. You will need to provide a brief rationale for your request, consult with your advisor and obtain your advisor's signature and return the completed form to the Registrar's Office.

The Registrar's Office will notify you and your advisor whether your request has been approved or denied.
