Travel Health and Safety Paperwork

Students: For all Study Abroad programs, including Hope College May/June Terms and Immersion Trips, please visit travel.hope.edu to register for a trip.

Please complete the forms below at least two days before the event. If you have questions about any Travel Health and Safety process, please contact Travel Health and Safety at travelsafe@hope.edu.

Step 1 – Itinerary Form 

Every trip requires this itinerary form to be filled out by the trip leader or advisor. This is the first required step in our new travel health and safety form process. Please fill out this form at least two days prior to the event.

The Travel Health and Safety team will share botht the itinerary and student information with the trip leader.

Step 2 – Student Form

Once an itinerary has been filled out, each student attending the event is required to fill out this student form. The student will need to select the trip they are going on and answer the remaining questions in this form. Please fill out this form at least two days prior to the event.
